Parent Marital Information

• Must be baptized, practicing Catholic (attends Mass regularly)
• Must be at least 16 years of age and cannot be the parent of the child
• Must have received all their Sacraments of Initiation: Baptism, Eucharist, and Confirmation
• If single, not cohabitating
• If married, in a valid marriage recognized by the Catholic Church

• Must be at least 16 years of age
• Must be a baptized Christian
• Must be living an upright life and committed to my faith
• Non-practicing Catholics may not act as a Christian Witness
