St. Jude's Baptism Checklist
Prior to Scheduling your child's Baptism, all paperwork must be turned in.
All forms need to be received at least two weeks prior to your child's baptism.
Download the Baptism Preparation Checklist to track the documents and requirements needed before scheduling your child's Baptism.

Baptisms are not scheduled prior to receipt of the following items:
- Baptism registration form
- Birth certificate of child
- Baptismal Godparent Form(s)
  - A letter from godparent's pastor may replace the pastor/delegate signature
- Christian Witness form (if applicable)
- Baptismal Preparation Class Certificates for both parents and Godparents. Baptism Preparation Class Sign-up
- OUT OF PARISH
  - Catholics form other parishes wishing to have their child baptized at St. Jude must present a letter from their pastor stating that they have permission to have their child baptized at St. Jude.
- Suggested Stipend of $75 payable to St. Jude Catholic Church. Donate
